Removed the original telephone jack/plate. I removed the plate and jack, unscrewed the Blue and Orange pairs from the telephone module (which used the old Red/Green/Black/Yellow colors) and replaced with 'snap-in' jack, and added an Ethernet 'snap-in' jack, then mounted the new wallplate:

Ethernet jack with 4 conductors on RJ45 (which limits speeds to FastEthernet 100Mbps; 8 conductors needed for Gigabit Ethernet). Because the Orange pair is already committed to the USOC phone standard, the Brown pair is used in place of the Orange:
